Please review before your upcoming visit with us:

At all times patients must comply with any existing CDC’s and/or Governor’s applicable executive order for facemasks. Our medical office team will wear masks due to close facial interaction and limited social distancing in exam rooms.  

Please plan to come in by yourself to protect other patients, unless a caregiver is needed.

Dear Grand Blanc Eyes PLLC patients,

An update to our current situation:

In March, Michigan’s Governor Whitmer declared a state of emergency as the state’s first positive cases of COVID-19 were identified. Our practice has been closed since March 23, 2020 given in response to executive order 2020-21, ordering all people in Michigan to stay home and stay safe. As of writing, the current executive order no. 2020-42 declaration places Michigan in a stay-at-home mandate extended to May 28, 2020.

For the last several weeks, Grand Blanc Eyes, PLLC has been preparing the office for a new environment when we return within our healthcare industry. We have taken action to adapt to these new processes and keep our patients and team safe. We currently plan to re-open safely with a very limited schedule, first priority to emergencies to avoid patient hospital visits, exam preference weight to diabetic retinopathy/macular degeneration/ocular cancer/glaucoma examinations, and last, procedures in place to eliminate the risk of disease spread. Our staff has held several meetings to review procedures and topics such as: face shields, acrylic front desk shields, provided N95 droplet masks and gloves, scrubs, Genesee county heath forms, visits by appointment only, screening and taking temperatures, socially distancing patients, UV disinfection policy, and hand sanitizer policy.

Please continue to stay safe practicing temporary separation, quarantine, and social distancing. All are encouraged to follow the recommendations from MDHHS and the CDC to protect themselves and slow the spread of COVID-19. Please feel free to ask Dr. Ann Walker or Dr. Agnone any questions.

Video 2: PPE Video topics reviewed: Arrive 15 minutes early, to Room 5. Washing hands, placing gloves, review masks, placement of mask, face shields, taking temperatures, daily employee form screening (see “Employee Return to Work Questionnaire and Acknowledgement.pdf” and “High Risk PT COVID19 Questionnaire Form – Over age 65 – After June 1st.pdf”), Scrubs worn and washed every day, and Hair. Video 3: Seeing scheduled appointments only Topics Reviewed: Please notice the signage upon entry outside and inside, Forms presented to Patients (see “Patient Screening Questionnaire Acknowledgement.pdf” , “COVID19 – Patient recommendations.pdf” , “High Risk PT COVID19 Questionnaire Form – Over age 65 – After June 1st.pdf”), Patients are directed to the reception area to fill out, the patient has his/her temperature taken if answered “No” to all four questions, Initial check-in can proceed after this screening takes place. Use the office in a circular flow to not back track patients. The recording is live and available to all staff for review as needed.

Video 4: Patient Walk-ins Topics Reviewed: Services will be provided by appointment only after phone/text screening; no walk-in patients except for prearranged emergency services or treatment. This include glasses selection or glasses adjustments. The signage reads exactly: “Enter by Appointment only. If you do not have an appointment: Please call (810) 694 – EYES (3937). NO exceptions.” At all times patients must comply with the Governor’s applicable executive order for facemasks and social distancing.
